All requests for any type of record for former students must be processed through the Caldwell County Schools Accountability
Office. This includes but is not limited to transcripts, medical records, immunization information, IEPs, etc.

Requests for information other than what is listed on the request form may be included in the special instructions portion of the form. Additional information may also be added there as well.

Avoid checking multiple boxes or you will be charged multiples of ten. Select the information type requested that is most applicable.

This is the most expedient method in getting the information request filled, which generally takes three to four business days.
