    Ms. Rose in the Science Office or download the forms below.

    Completed forms must be returned to Ms. Rose by Friday, June 1st.  

    LATE FORMS WILL NOT BE ACCEPTED!

    Information for Seniors, Parents, and Guests

    Seniors in order for you to attend Project Graduation, you and your parent/guardian must complete a Senoir Release/Consent form and return it to school by Friday, June 1st to Ms. Jessica Rose, Science Department. 

    Even if you are unsure that you will attend this function, it is to your advantage to submit your form, keeping the option available to you.      

    Seniors may invite one guest.  Tickets for guests are $10.00 each, payable by Friday, June 3rd, to Ms. Jessica Rose,Science Department.  If your guest is not a senior in Caldwell County, he/she must complete a GUEST RELEASE/CONSENT form and submit it along with your form.  Your personal invitiation(s) will be sent upon receipt of these fully completed and signed form(s).  The invitiation, along with photo identification (driver's license) will be required for admittance for you and your guest.
